We are doing our best to add patients from our waitlist but given the shortage of family physicians in our community we are struggling to keep up with the demand. Please write letters to your MPP about making sure there are enough Family Doctors to service Canadians properly for now and the future....thank you.
- NEW PATIENT Intake Form: If you care looking to join our practice as a new patient, please come into the office and ask for an Intake Form. Fill it in completely, and give it back to our office to be added to our waitlist. We will call you with an appointment time once space is available. Note: Currently we are full and will not be accepting any intake forms at this current time.
- MEET and GREET APPOINTMENT: Once we receive your intake form, when a spot opens up in one of our practises, our administrator will call you to book a Meet an Greet Appointment. At this appointment, we will go over the general way our office works and you will be able to ask questions that are important to you. This is not a clinical appointment and we will not be seeing you for medical care, but for the sole purpose to meet one another and to establish if it is a good fit on both sides.
- Consent to Release Medical Records Form: Once we have met you and have agreed to become your family physician, you can download the link below "Consent to Release Medical Records", open the file, print the form and fill it in, (please circle the physician whose practice you are joining) and then mail or fax the form to your current family doctor so that your medical records can be safely transferred to our office. Please do not transfer your records to our office until after your Meet and Greet Appointment and the physician has agreed to take you on a patient. By doing this step before the Meet and Greet, you may lose your family doctor before securing a new one.
- CONDUCT and AGREEMENT LETTER Form: If we have met and you and we agree that we are a good fit to become your family physician, you will need to read, agree to, and sign the Patient Conduct and Agreement Letter below. You may print it off and sign or come to the office to read and sign form.
Once the above documentation is completed and your file has been transferred from your past family doctor,
we are able to take over your care as your new family physician.
