We are doing our best to add patients from our waitlist but given the shortage of family physicians in our community we are struggling to keep up with the demand. Please write letters to your MPP about making sure there are enough Family Doctors to service Canadians properly for now and the future....thank you.
- NEW PATIENT Intake Form: If you care looking to join our practice as a new patient, please download this link below, open the file, print the form, fill it in completely, and fax it back to our office or drop it off in person to be added to our waitlist. We will call you with an appointment time once space is available.
- Consent to Release Medical Records Form: Once we have met you and have agreed to take you under our care as your family physician, you can download this link below, open the file, print the form and fill it in, (please circle the physician whose practice you are joining) and then mail or fax the form to your current family doctor so that your medical records can be safely transferred to our office. Please do not transfer your records to our office until after your Meet and Greet Appointment.
- CONDUCT and AGREEMENT LETTER Form: If we have met and you and we agree that we are a good fit to become your family physician, you will need to read, agree to, and sign the Patient Conduct and Agreement Letter. You may print it off below and sign or come to the office to read and sign form.
